In the event of two people nominating the same person, who subsequently joins Spiderweb, the person who submits first (determined by the time and date on the emailed nomination) shall be deemed the nominator.

How to Submit:

1.  Enter your Name & Email Address in the boxes below. Enter the Name and Address and telephone number of the person you wish to nominate in the box provided.
2. Click on the "Send Nomination" button.  If you make a mistake, click the "Clear form and start again" button.
